THE CARING ABOUT CARERS AWARD 2012
Do you look after a relative or friend who is sick, disabled, frail or has a problem with alcohol and/or drugs?
If yes, then you know how help from your GP and his or her practice team can make all the difference.
The Royal College of General Practitioners, in partnership with the Carers Trust, is looking to find GPs who provide the best support to unpaid carers.
Nominate your GP or surgery for this special award by writing to us about how they support you in your caring role.
Remember to include:
• your name, address, telephone number
• your doctor’s name and surgery contact details
• how they support you in your caring role.
The closing date for entries is: 22 June 2012
